Records Requests

1. Print out the Records Release Form.
2. Complete the form and sign it. 
3. Mail or fax the form to the Health and Wellness Center. Fax 610-526-7365
4. Alternatively, you may send the completed form as an email attachment to Susan Fitzgerald, RN, sfitzgeral@brynmawr.edu.

Health and Counseling Services maintain student health records for 7 years after the date of graduation.
